CRC Daily: Where to get a Covid test in Costa Rica (official list)
A map and list of where to get a coronavirus test in Costa Rica.
Tourists returning to the United States, Canada, and several other countries must get a coronavirus test before departing Costa Rica.
In response to these requirements, Costa Rican authorities have compiled a list of labs that promise PCR test results within 48 hours, faster than the 72 hours typically required for international travel. Labs can set their own prices, but the Tourism Board recommends under $100 in the San José area, and under $150 elsewhere.

In the above link, labs with a white background offer PCR tests only; labs in green offer antigen tests only; and labs in orange offer both.
Both Costa Rica airports offer on-site Covid testing as well.
Below is a screenshot of the full list as it appeared on January 27, 2021. The table is sorted by region.
Costa Rica itself does not require a negative test from travelers.
